We believe strongly in preventive medicine for kids. This giveaway helps raise awareness for well-checks and rewards parents for keeping their kids current on preventive care.

Who is eligible for this giveaway?
Utah families that live in one of the counties we serve are all eligible. See the official rules to check if your county qualifies.

How many times can I enter?
Each family can complete the entry form once, but you’ll have the opportunity to earn multiple entries during that process.

Do I need to be a Families First Patient to enter?
No, you don’t.

How will the prize be awarded?
We’ll only contact the selected winner by phone. The prize will be claimed in person. We will never ask for your credit card or bank information.
